Thank you for enquiring about becoming a substitute Educational Assistant in Southwest Horizon School Division.

Please supply us with the following:

A completed Substitute Educational Application form

A completed Application for Direct Payroll Deposit form

Current Criminal Record Check and Child Abuse Registry Check

Completed TD1 Federal and TD1MB Forms

For the record checks you need to provide a current criminal record and child abuse check (please see our website Policy GAD for futher information if required).

If you already have these records, we will accept copies, as long as they are less than 6 months old.  If you do not have any records you need to obtain a Criminal Record Check by filling in the form at your local RCMP/constabulary office.

A Child Abuse Registry self-check can be done by downloading the forms from the website.
